How Effective is Math Comment Spam Protection?

I have been using Math Comment Spam protection for quite some time. It is a simple WordPress plugin, yet awesome spam protection results.
The Math Comment Spam plugin running well until my readers told me they can’t give comment. I checked on the plugin no problem found, at last give up the plugin because cannot find the root cause.

I thought Akismet plugin should be able block those spam. Yes, it helps to identify spam comment and trackback. But it is annoying to go through huge amount of spam comments to find out which not spam.
Spam comments come in non-stop everyday, highest one get 141 spam comments in a day!
